Sure, make the form (leave the fields empty) and Save it.
Send it to anyone and as long as they have (minimum) Adobe Reader, they can fill in the fields, and do a "Save as. "
That should be it or am I missing something in your request?

Based on how I asked the question, you probably didn't miss anything! I was trying to be succinct. It would be very helpful if I could save a completed fillable pdf as another pdf that embeds the data in the fillable fields; then I could append it to another pdf. If there is an instance of the completed fillable and an instance of the blank fillable--as happens--I can't append both of them to the same pdf. Confusing at best but I know what I'm trying to accomplish! Creating a portfolio doesn't work either for me.
